Projects A & B:
PROJECT A: Choose two patients below. Diagnose the following patients. Explain how you would counsel and treat the patient. Include a professional presentation including an explanation of the diagnosis and the care plan.
Case 1
James and Amanda are first time parents. They are both 26 years old. Their son, Adam, is 8 weeks old. Amanda and James bring Adam to your office. Both parents look unwell and Amanda is weepy in the waiting room. You saw Adam at his 6-week check-up and he was healthy. Today, James and Amanda confide in you that they are not sure it was a good idea for them to become parents. They feel they took on more than they can handle and they wonder if something is wrong with Adam. Shortly after the 6-week checkup Adam started crying a lot. He usually cries, nonstop, from 10pm until 2am. This happens every day and has been going on for the last two weeks. James and Amanda have no family in this area and they are the first of their friendship group to become parents.
Case 2
You’ve been called to the hospital emergency room for a consultation. The regular ER pediatrician is away on vacation and your diagnostic help is needed. Baby Lizabeth, who is six weeks old, is presented by her mother, who is worried about her high temperature. The baby, who was normally alert for many hours of the day, has been lethargic and fussy. When her mom finally got her up from sleeping she noticed that the baby felt hot to the touch. Mom took a rectal temperature and it was 101 degrees. When asked for a record of the baby’s immunizations, the mother reports that she is against immunizations because she believes they cause autism. The child has not been given any immunizations.
Case 3
Anne is 39 weeks pregnant with her first child. The pregnancy has been uncomplicated. This morning she noticed some bleeding after she urinated. She assumed it was her mucus plug and was excited that she would be going into labor soon. Shortly after that she started to feel nauseous, which she thought was unusual, since she hadn’t felt that since her first trimester. She wondered if labor was starting right away. Her baby was moving less than usual and she started to feel some abdominal pain. “Oooh! I’m definitely in labor!” she thought and she called her midwife. Anne told the midwife that she had been bleeding some and that she assumed it was because she lost her mucus plug. She also told the midwife she was having contractions and felt nauseous. The midwife asked her how far apart the contractions were. Anne responded, “It’s like one long contraction. There’s no space in between them.” “Meet me at the hospital,” the midwife said. Why is the midwife worried?
Case 4
Michael is a 15 year old boy whose father has brought him to the pediatrician. For the past two weeks, Michael has had swollen glands in his neck. He has also been tired and doesn’t feel like eating. In the last two weeks he has lost 6 pounds. Michael says his throat does not hurt. He does say he has started sweating at night a lot and thinks he has been running a high fever this week. His father had taken his temperature every day. It ranged from 101 to 103. At your office visit Michael does not have a fever. Michael’s father is extremely distressed and says that he would have brought Michael to the doctor sooner but he is a single father and has no health insurance.
Case 6
Janessa brings her 6 week old son, Tyric, to the emergency room. Tyric was born an albino, which was unusual enough, but Janessa reports that she is very worried about his health. Tyric has been experiencing tremors and has been getting nosebleeds. Janessa reports that Tyric’s older brother accidentally smacked him with a bouncy ball and Tyric did not seem hurt by it, in fact, he didn’t seem to feel it at all. Janessa wants a specialist to explain what is going on with her baby. She is very concerned.
Case 7
Jennifer is a 29 year old female who comes to your office complaining of abdominal pain and feeling pressure in her belly. She says that even when she eats very little she feels full and cannot eat anymore. This thinks this is a
good thing since she is quite overweight and would like to lose weight. Jennifer has also noticed that she has
had to urinate more often and it is now interrupting her work. She has to leave her desk to use the bathroom every hour. Jennifer’s mother died from breast cancer when she was 50 years old. There is no other family history of illness. Jennifer does not have children and has not tried to become pregnant yet. She uses condoms for birth control.
Jennifer is 5’5” tall and weighs 180 pounds.
Case 8
Alan is a 38 year old male who comes to your office complaining of a constant cough, feeling short of breath and feeling like his chest is tight. Lately he has been feeling more tired than usual and has started wheezing. Alan is a smoker and has recently been trying to quit. He currently smokes 3 cigarettes a day but has smoked a pack a day for the last 15 years. When asked about his diet, Alan reports that he is “too busy to worry about eating healthy.” Alan reports that he does not exercise very much. He is not overweight but says he is “too tired to exercise. Even walking wears me out.”
Case 9
Betty is a white, 60 year old woman. She is 5 feet tall and weighs 105 pounds. Betty has suffered from asthma her
whole life and takes medication to control it. Betty has been transported to the emergency room after a fall in
her home. Betty has spinal compression fractures as well as fractures in her wrist, hip and pelvis. She is very embarrassed and explains that she needs to recover quickly so that she can get back to work as a teacher. Betty explains that her job is very busy and that it takes all of her time and energy to do her work. She says that lately she hasn’t even found time to eat or take her vitamins.
Case 10
Charlie is a 38 year old white male. He is 6 feet tall and weighs 184 pounds. Charles is in the emergency room after
getting into a minor car accident. He has suffered some bumps and bruises but is otherwise not injured. Charles is very upset though. He reports that he has never been in a car accident before and that the accident was his fault.
He also complains that he has been forgetting things lately at work and at home and this has been causing stress between him and his boss as well as him and his wife. He says he is exhausted all the time. He also reports a “strange feeling in my legs at night”.
Case 11
Denise is a 31 year old African American woman who comes to your office complaining of pain before and after menstruation. She says that the pain is disabling and often occurs after urination as well. She is also experiencing
chronic lower back pain. Denise reports that she has no children but would like to become pregnant.
Her husband and she have been trying for eight months to get pregnant with no success. Her period comes
regularly in a 26 day cycle and it lasts for 8 days. She got her first period at age 10.
MATERIALS NEEDED: Computer access.
PROJECT B: Suturing: Students are presented with a “patient” who has been injured. Students learn to provide appropriate suturing to close a wound. Teacher should buy chicken with bone still in it and cut a deep slice through the skin and meat of the chicken. The cut should go deep – almost to the bone. The chicken is then the patient that needs to be sutured. Students should teach themselves suturing using online instructions found on youtube (several good choices available including one that shows suturing into a banana. Watch out for graphic ones though – many show real surgeries or people doing “at home surgery”). After learning suturing, the student will practice on felt material and then move on to the raw chicken representing human flesh.
MATERIALS NEEDED: Chicken, latex gloves, doctors mask, student suturing, YouTube access.
